DESCRIPTION:

Provide personal care and related assigned services to terminally ill patients in the home in accordance with an established physician plan of care and under the supervision of a registered nurse case manager as dictated by state. Directly involved with caring for patients and promoting comfort on a daily basis. Observe the patient, report observations and document observations and care performed as set out by the plan of care.

QUALIFICATIONS:

Proof of state certification
Proof of auto insurance
CPR certified
Minimum of 1 year of direct patient care experience in a home care setting
Compliance with accepted professional standards and practices
Valid drivers license with maintenance of good driving record and an automobile that is insured in accordance with state
Good physical stamina and mental health
Certification including a negative TB skin test and/or CXR
Satisfactory references from employers and/or professional peers
Demonstrate excellent observation, good judgment and communication skills. Ability to provide good written documentation in a timely manner
Ability to read and comprehend simple instructions, write simple correspondence
